Data Deletion Request

If you have communicated with FBM Clinic via Instagram, Facebook, or other platforms and would like your data deleted from our system, please fill out the form below.
Important: Since this panel is an internal tool, you do not have an account here. We use your registered email or phone number to verify your data.
How the Process Works
- 1. Fill out the form below.
- 2. If your record exists in our system, a verification message is sent to your email/phone.
- 3. Confirm your request by clicking the verification link or entering the code.
- 4. After confirmation, your data will be deleted within 30 days.
- 5. You will be notified when deletion is complete.
